The Registrar's Office regularly assists alumni and graduating students with documentation for licensure. Any forms that require completion can be emailed to email@example.com.
As each state has specific requirements, it is recommended that alumni and graduating students contact the appropriate board with questions regarding the licensure process.
If additional documentation is required when submitting a licensure application (ex: official transcript, notarized diploma, english translation), the alumni or graduating student must complete a separate request for each. If a notarized copy of the diploma or english translation is required, complete the MCG Graduate Document Request and email firstname.lastname@example.org a photocopy of the diploma.
For alumni using the Federation of State Medical Boards (FSMB) - Federation Credentials Verification Service (FCVS) application, upload a copy of your MD diploma and an electronic copy of your official transcript.
