The Registrar's Office regularly assists alumni and graduating students with documentation for licensure.  Any forms that require completion, can be emailed to registrar@augusta.edu.

As each state has specific requirements, it is recommended that alumni and graduating students contact the appropriate board with questions regarding the licensure process.

If additional documentation is required when submitting a licensure application (ex: official transcript, notarized diploma, english translation), the alumni or graduating student must complete a separate request for each.  If a notarized copy of the diploma or english translation is required, complete the Medical Student/Graduate Document Request Form and email registrar@augusta.edu a photocopy of the diploma.  

For alumni using the Federation of State Medical Boards (FSMB) - Federation Credentials Verification Service (FCVS) application, upload a copy of your MD diploma and an electronic copy of your official transcript.
