Clinical Experience Block 5

The PCL Clinical Experience includes: Telemedicine Patient Experience Session, Clinical Procedures Session, and Ultrasound Session. 

BLOCK 5 OBJECTIVES AND ASSESSMENTS

1:00 - 2:45PM

Telemedicine Encounters (1:00 or 1:25 start) 

  • The interviewer will be conducting the encounter while the scribe observes
  • All visits with real patients will be done in EPIC. 
    • Review how to login and set up EPIC
  • Complete the SOAP Presentation Debrief Qualtrics form (Interviewer) with a faculty member during your debrief session
  • Complete Telehealth Note Feedback Qualtrics (EVERYONE)

Clinical Procedures (2 groups will go at a time) 

  • Practice interpreting and troubleshooting EKGs
  • Do not begin ultrasound portion until completing the telemedicine debrief and the EKG case. You may work on your patient notes while waiting or watch the post work video case/EKG which is needed to answer the post quiz questions.

2:45 - 4:30PM

Cardiac Ultrasound

  • You will be obtaining ultrasound images of the heart and IVC on your partner.
  • Pull up the Ultrasound Debrief Survey prior to scanning. This will contain example images and videos on how to obtain the ultrasound images  
  • Consent your partner prior to obtaining images. SPs are available to be scanned as well. 
  • Have your images reviewed by a faculty member and submit the debrief survey prior to leaving clinic.  *Faculty must watch you obtain 1 of your 4 images.* 
  • Email the IVC longitudinal imaLge to yourself for the post-quiz in D2

Telemedicine Patient Encounter

· REFRESH

  • Review PCL Note Template with additions specific for primary care and chronic conditions and Example Note that follows this template 
  • Review Telehealth Exam

· PREWORK

  • For those who have seen their patient before review your patient's medical history
    from your prior notes and prepare a tentative plan for your telehealth visit. Take
    into account you will not have EMR access to be able to review new events since
    your last visit. This information will need to be obtain during your interview. For
    those seeing a new patient develop a plan for an initial encounter with a patient
    when unable to review chart information.
  • Review the SP patient information in the case your patient does not show
  •  ‘Follow up visit without complaint’ Chronic issue interview/documentation

· POSTWORK

  • Recorders must complete your patient's note by 5:00 PM the day after your patient encounter and sign/submit your note to Clinical Experience Preceptor. This must be done through EPIC.  Please review this video if you are unsure how to submit your note through EPIC.  
  • If you have an SP encounter, you should send your note via email to your Clinical Experience Preceptor and copy your partner.  
  • CHANGE: EVERYONE (interviewer and recorder) must fill out Telehealth Note Feedback Qualtrics as soon as you finish debriefing with PCE faculty.   Completion of this survey is required for all telemedicine visits, regardless of whether the encounter was with a real patient or a standardized patient.  
  • Complete the Clinical Experience Post-Visit Survey
  • If any of the above is not completed on time you will lose professionalism points.

  • Follow up CBL Case Synopsis (Your team will see this patient during your procedure time)

· POSTWORK (watch prior to completing your post-work quiz)

  • Vitals are Vital Podcast Block 5 Video (Reliable Tests) - covers one case/ EKG and Ultrasound match
  • Case/ EKG and Ultrasound Matching Video  (will assist with post quiz questions)
