Step 1 The contractor's office personnel must request access for all contractors by submitting an Animal Facility Access Request. Email addresses are required for each individual to be cleared. The Augusta University employee responsible for the contractor must be listed as PI/ Supervisor.
Step 2 All contractors must complete and return the Occupational Health Services Questionnaire for Contractors who enter Augusta University Animal Facilities to ANIMALOHP@augusta.edu. This tool determines if contractors must be evaluated by employee health.
Step 4 Laboratory Animal Services training is required, once all prior steps (1-3) are completed and clearance is granted by Occupational Health/ Employee Health. The LAS Training Coordinator will contact individuals to schedule this training. The Augusta University Department requesting access will be notified of completed contractor clearance.
