Family & Community Medicine Center Patient Appointments

If experiencing a medical emergency, immediately call 911 or report to the nearest emergency room.

New or Established Routine Patient Appointments 
Routine appointments include annual physical exams and health assessments, school and sports physicals, women's health assessments and evaluation to include pregnancy management, and cervical and breast cancer screening, and management of chronic medical problems (i.e., Diabetes, Heart Disease, Asthma, Obesity, Hypertension, Elevated Cholesterol, etc).

Arrive 15 minutes prior to appointment time, bring insurance card, picture ID, copayment, and list of current medications.


Same Day Patient Appointments

The clinic provides same day appointments for established patients that have acute non-emergent problems such as simple injuries and acute signs and symptoms (i.e., Rash, Urinary Tract Infections, Fever, or Changes in the Control of Chronic Medical Problems).
