Fellowship Application

PREREQUISITES:

1. Completion of a 3 year US Internal Medicine Residency.
2. Applicants must have passed steps 1, 2, and 3 USMLE to be considered.
3. Applicants with visas pending can not be considered.
4. Applicants with H-1 visas can not be considered.
5. J1 visa, permanent residents, and US citizens can be considered.
6. Interviews are offered based on visa in hand at the time of application.
7. We will not review incomplete applications.

 

HOW TO APPLY:

We accept applications by ERAS only. We participate in the National Resident Matching Program.

 

APPLICATION REQUIREMENTS:

1. Completed and signed application
2. CV
3. Personal statement
4. Current Picture
5. Copy of USMLE Certified Transcript of Scores (or COMLEX)
6. Copy of medical school diploma
7. Copy of medical school transcript
8. Copy of J-1 Visa or Green Card if not a US citizen
9. Copy of ECFMG certificate where applicable
10. Three (3) letters of reference, one from your current program director
11. Dean's letter
