
YourTown Health—formerly Palmetto Health Council, Inc. (PHC) was formed in 1978 by a group of citizens in Palmetto, Georgia, to increase access to health care services for this rural community. PCH secured federal funds to establish a Federally Qualified Health Center, Community Medical Center of Palmetto, in April 1980. In 2013, to better describe its services, Palmetto Health Council was renamed YourTown Health.
YourTown Health is a non-profit FQHC network of seven Community Health Centers serving the West Georgia counties of Meriwether, Pike, Lamar, Carroll, Coweta, and South Fulton for over 40 years. This FQHC network of health centers offers pediatric and adolescent, family practice, internal medicine, obstetrics and gynecology, dentistry, school and occupational health, telehealth, urgent care, and pharmacy services.
YourTown Health participates in the National Health Disparities Collaborative for five frequently occurring chronic diseases to measure the quality of care given to its patients based on health outcomes compared to those nationally, to assess and improve quality, and produce better health outcomes. This FQHC is recognized as a “Patient Center Medical Home” with an interdisciplinary team of (physicians, nurses, nurse practitioners, physician assistants, dentists, dental hygienist, pharmacist, and licensed clinical social workers) providing comprehensive and coordinated approaches to the delivery of primary care services with continuous quality assessments and improvements to meet the health care outcome needs of the “whole” patient.
YourTown Health’s team of healthcare professionals provides comprehensive, continuous, and personalized medical services to every patient who walks through their doors with a focus on community well-being, where individuals and families can receive healthcare irrespective of their ability to pay.
YourTown Health's interprofessional comprehensive primary care clinical settings provide learning opportunities for MCG students to experience and participate in the delivery of care to underserved communities and populations. As an MCG FQHC/CHC Educational Affiliate, they will provide a variety of student-required and elective educational opportunities under the guidance of physician staff members who also serve as MCG Community Clinical Faculty.
