
East Georgia Healthcare Center (EGHC) is a non-profit community-owned Federally Qualified Health Center, governed by a local board of directors, the majority of whom are active patients.
The health center’s Mission is “To help all people live healthy lives by providing quality, primary and preventive health services”. Beginning in 1994 with a site in Swainsboro, Georgia, which now houses its corporate office, EGHC now serves the 14 counties of Emanuel, Bulloch, Treutlen, Toombs, Tattnall, Jenkins, Candler, Appling, Wayne, Montgomery, Jenkins, Jefferson, Screven & Evans.
The clinical services provided by EGHC include Adult and Pediatric Medicine, Dental, Behavioral Health, Pharmacy, Optometry, Telehealth, PRN, Acupuncture, and Chiropractic care. Three mobile healthcare units are employed to expand access to services outside of CHCS’s clinical offices to provide “Health Checks” for populations not normally reached because of logistical or other challenges. EGHC is supported by an interprofessional team of providers, including physicians, nurse practitioners, physician assistants, and licensed clinical social workers.
This FQHC also provides support for school-based clinics and the Georgia Farmworker Health Program. EGHC is a Patient-Centered Medical Home (PCMH) which promotes high-quality care standards that emphasize the use of systematic, patient-centered, coordinated, team-based care that supports access, quality assessment and improvement, and patient involvement. EGHC is a participant in a project, along with several other Georgia FQHCs, to develop a collaborative Teaching Health Center-sponsored Family Medicine Residency Program, which will engage area hospital health system partners.
EGHC clinical platforms provide multiple learning opportunities for a variety of required and elective clinical experiences for students as a Medical College of Georgia (MCG) Educational Affiliate. These learning opportunities are supervised by members of the EGHC’s physicians who also are MCG Community Clinical Faculty members and are facilitated by other members of the health center’s interprofessional team of providers.
