
Curtis V. Cooper Primary Health Care, Inc. is a federally qualified health center (FQHC) that provides quality primary and preventive care. We are available for all your health and wellness needs. We are recognized by the National Committee of Quality Assurance (NCQA) as a Patient Centered Medical Home Level 3 since 2013 and have an active quality assurance/improvement program all year round.
We offer same-day appointments for those your immediate needs. We provide an array of services through our various locations from women’s health, pediatrics, behavioral health, and dental care. You may visit any one of our locations throughout the Savannah-Chatham county and surrounding areas.
We also offer telehealth services for patients who want to remain in the comfort of their own homes, but still access medical and behavioral health services.
