Radiology & Imaging - Credentialing & Verifications

The Department of Radiology and Imaging provides credentialing, verifications, and documentation for current and former residents and fellows. Please include the following information with each verification request:

  • Release of information form signed by the current or former resident or fellow
  • Dates of training
  • Trainee's date of birth
  • Name during the time of attendance
  • Preferred method of response to the request

Fee Schedule

Verification requests can be sent by email to radresidency@augusta.edu or via mail to Augusta University, Department of Radiology and Imaging, Radiology Residency, 1120 15th Street, BA 1404, Augusta, Georgia 30912.  

NOTE:  Requests cannot be accepted via telephone.

SERVICE FEES

Verifications for residencies and fellowships are subject to the following fees, payable by check.  We do not accept credit cards.  The fees cover the costs associated with the retrieval and processing of verifications and references. 

Verifications of graduates who are 0 – 2 years post-program completion No fee
Verifications of training dates, program specifics on training including procedures, commentary on performance, RAM licensure, etc., on institution or agency-provided form for graduates 2 – 10 years post-program completion $50
Verifications for graduates 10+ years post-program completion $75
Case logs $35 fee in addition to verification fee
Verifications requiring notarization $5 fee in addition to verification fee
Malpractice claims history and insurance verification Submit your request to Legal Affairs here

NOTE:  Verification services are rendered after your payment is received by our office.  If you require an invoice or W-9, please indicate this in your request. Please allow 7-10 business days for verifications to be completed.  If you have questions or concerns about your verification request, please contact our residency program office at 706-721-2076.
