To expedite handling of your request for veterinary supplies or services please complete the form below. You will receive acknowledgment of your request within 1 business day. Supply requests fulfilled by DLAS are subject to a 20% service fee that will be included in the total cost.

• If you are requesting a veterinary technical service, the service must be scheduled 8 days in advance.
• If you are submitting a request for transport boxes please include:
  • Number of Boxes Needed
  • Box delivery Location
  • Number of dividers (maximum of 3/box)
