Child Safety

Your baby must ride in a car seat all the time, including the trip home from the hospital.
This is required by law until the child is 4 years old or 40 pounds. After that you should use the car's seat belt.


The WIC Clerk from M&I is available Monday-Friday and will contact you during your hospital stay to put your baby on WIC, if you live in the East Central Health District. If you are not seen by the WIC Clerk during your hospital stay, contact your local health department to see if your baby is eligible for WIC.

Birth Certificates

If you are not married to the baby's father and want to give the baby his last name, both parents must sign a Paternity Acknowledgement Form which may be obtained at the hospital and the father is required to provide a picture ID at the time of signing. A copy of the birth certificate can be obtained after the baby is 2 months old from:

The fee is $10.00 for the first copy and $5.00 for each additional copy, if ordered at the same time. When requesting my mail, please include:

■ Baby's full name and date of birth
■ Mother's and Father's name
■ Name of hospital where baby was born

A request for a social security card for the baby can be made when the birth certificate is completed in the hospital.


These should be started when your baby is 1-2 months old. Contact your local health department for an appointment.

Postpartum Checkup

Your "after the baby" check-up is important. You will be mailed an appointment for this. Be sure to keep this appointment. You will get birth control information and may qualify to stay on the WIC program if you keep this appointment. It is not necessary to bring your baby with you for this appointment.
