Interventional Pulmonology Fellowship Application

PREREQUISITES:

1. Completion of a 3 year US Internal Medicine Residency.
2. Completion of a 3 year US Pulmonary and Critical Care Fellowship or 2 year US Pulmonary Fellowship.
3. Applicants must have passed steps 1, 2, and 3 USMLE to be considered.
4. Applicants must be board-certified by the American Board of Internal Medicine in Pulmonary Disease.
5. Applicants with visas pending can not be considered.
6. Applicants with H-1 visas can not be considered.
7. J1 visa, permanent residents, and US citizens can be considered.
8. Interviews are offered based on visa in hand at the time of application.
9. We will not review incomplete applications.

9. Copy of ECFMG certificate where applicable
10. Three (3) letters of reference, one from your current program director
11. Dean's letter
